Introduction Outbreaks of serious communicable infectious diseases remain a major global medical problem and force healthcare workers to make hard choices with limited information, resources and time. While information regarding physicians’ opinions about such dilemmas is available, research discussing students’ opinions is more limited. Methods Medical students were surveyed about their willingness to perform medical procedures on patients with communicable diseases as students and as physicians. Students were asked about their opinions regarding the duty to treat in such cases. Results (...) 74% of respondents felt that by deciding to enter medical school they were morally obliged to treat any patient despite the risks. Students’ willingness to treat as physicians is significantly higher than their willingness to treat as students. HIV was significantly the most tolerated disease with respect to performing mouth to mouth resuscitation. Among preclinical students, we found that willingness to treat during the later years is significantly greater than during the earlier years. Among clinical students, the opposite was observed. Discussion Students’ greater willingness to treat as physicians is mostly attributed to perceptions of higher obligations as a qualified doctor. There is greater but not total willingness to perform resuscitation on patients with HIV relative to other diseases. The increased willingness of preclinical students and the decreased willingness of clinical students both emphasise the importance of patient–physician communication and ethics studies during medical school. Sometimes people desire that their lives go badly, take pleasure in their lives going badly, or believe that their lives are going badly. As a result, some popular theories of welfare are paradoxical. I show that no attempt to defend those theories from the paradox fully succeeds.

Several recent publications in biomedical ethics argue that organ donation is generally morally obligatory and failure to do so is morally indefensible. Arguments for this moral conclusion tend to be of two kinds: arguments from fairness and arguments from easy rescue. While I agree that many of us have a duty to donate, in this article I criticize these arguments for a general duty of organ donation and their application to organ procurement policy. My concern is that these arguments neglect (...) the role that trust plays in contemporary organ transplant policies and in differential rational attitudes toward donation. Recognizing donation as an achievement of trust, and acknowledging the warrant of many people's rational distrust or withheld trust in medicine, I argue, should have significant implications for the ethics of organ procurement. Democracy is commonly associated with political equality and/or majority rule. This essay shows that these three ideas are conceptually separate, so the transition from any one to another stands in need of further substantive argument, which is not always adequately given. It does this by offering an alternative decision-making mechanism, called lottery voting, in which all individuals cast votes for their preferred options but, instead of these being counted, one is randomly selected and that vote determines the outcome. This procedure (...) is democratic and egalitarian, since all have an equal chance to influence outcomes, but obviously not majoritarian. (shrink)

ABSENCE is the claim that, if a symbol appears on a map, then absence of the symbol from some map coordinate signifies absence of the corresponding property from the corresponding location. This claim is highly intuitive and widely endorsed. And if it is true, then cartographic representation is strikingly different from linguistic representation. I argue, however, that ABSENCE is false of various maps and that we have no reason to believe it is true of any maps. The intuition to the (...) contrary results from mistaking what a map simply conveys for what it literally represents. (shrink)

Whilst certain verbs may appear in both the intransitive inchoative and the transitive causative constructions (The ball rolled/The man rolled the ball), others may appear in only the former (The man laughed/*The joke laughed the man). Some accounts argue that children acquire these restrictions using only (or mainly) statistical learning mechanisms such as entrenchment and pre-emption. Others have argued that verb semantics are also important. To test these competing accounts, adults (Experiment 1) and children aged 5–6 and 9–10 (Experiment 2) (...) were taught novel verbs designed to be construed — on the basis of their semantics — as either intransitive-only or alternating. In support of the latter claim, participants' grammaticality judgments revealed that even the youngest group respected these semantic constraints. Frequency (entrenchment) effects were observed for familiar, but not novel, verbs (Experiment 1). We interpret these findings in the light of a new theoretical account designed to yield effects of both verb semantics and entrenchment/pre-emption. (shrink)

Previous studies have shown that children retreat from argument‐structure overgeneralization errors (e.g., *Don’t giggle me) by inferring that frequently encountered verbs are unlikely to be grammatical in unattested constructions, and by making use of syntax‐semantics correspondences (e.g., verbs denoting internally caused actions such as giggling cannot normally be used causatively). The present study tested a new account based on a unitary learning mechanism that combines both of these processes. Seventy‐two participants (ages 5–6, 9–10, and adults) rated overgeneralization errors with higher (...) (*The funny man’s joke giggled Bart) and lower (*The funny man giggled Bart) degrees of direct external causation. The errors with more‐direct causation were rated as less unacceptable than those with less‐direct causation. This finding is consistent with the new account, under which children acquire—in an incremental and probabilistic fashion—the meaning of particular constructions (e.g., transitive causative = direct external causation) and particular verbs, rejecting generalizations where the incompatibility between the two is too great. Some people claim that evolution is "just a theory". Do you know what a scientific theory really is? Just a theory is an overview of the modern concepts of science. A clear understanding of the nature of science will enable you to distinguish science from pseudoscience (which illegitimately wraps itself in the mantle of science), and real social issues in science from the caricatures portrayed in postmodernist critiques. Some jurisdictions that allow assisted dying require participating patients to have a terminal illness. This includes all Australian and US states where assisted dying is allowed. 1 Philip Reed 2 argues that this requirement constitutes discrimination against the dying. As Reed 2 argues: ‘assisted death laws that limit their services to the dying discriminate against them because death is offered to them to solve their problems’. This discrimination could take two forms: (1) via harm to dying patients as a group (...) as a result of the existence of assisted dying programmes; (2) via a negative message that such programmes communicate about whether the lives of dying patients are worth living. Assisted dying programmes are not unjustly discriminatory in either sense so long as we assume that unjust discrimination requires harm to the group in question. The aim is to find out what theirbasic convictions regarding nature and sustainabilityare, and to analyze the relation between theseconvictions and the actual choices they make in theirfarming practice. Millianism and Fregeanism agree that a sentence that contains a name expresses a structured proposition but disagree about whether that proposition contains the object that the name refers to (Millianism) or rather a mode of presentation of that object (Fregeanism). Various problems – about simple sentences, propositional‐attitude ascriptions, and sentences that contain empty names – beset each view. To solve these problems, Millianism can appeal to modes of presentation, and Fregeanism can appeal to objects. But this raises a further problem: (...) namely, to explain why the proposition expressed by a sentence that contains a name matters in some cases but not in others. (shrink)

New reproductive technologies allow parents some choice over their children. Various moral principles have been suggested to regulate such choices. This article starts from a discussion of Julian Savulescu's Principle of Procreative Beneficence, according to which parents ought to choose the child expected to have the best quality of life, before combining two previously separate lines of attack against this principle. First, it is suggested that the appropriate moral principles of guiding reproductive choices ought to focus on general wellbeing rather (...) than prioritizing that of the child and, second, that they ought to be non-maximizing. Though neither of these suggestions is entirely novel, combining them results in a new, and arguably more plausible, principle to regulate procreative choices, which I call the Principle of Generalized Procreative Non-Maleficence. According to this principle, the primary obligation on parents is not to cause harm to other people through their reproductive choices. (shrink)

Abstract:Recently the scope of protections afforded those healthcare professionals and institutions that refuse to provide certain interventions on the grounds of conscience have expanded, in some instances insulating providers (institutional and individual) from any liability or sanction for harms that patients experience as a result. With the exponential increase in the penetration of Catholic-affiliated healthcare across the country, physicians and nurses who are not practicing Catholics are nevertheless required to execute documents pledging to conform their patient care to the Ethical (...) and Religious Directives for Health Care Services as a condition of employment or medical staff privileges. In some instances, doing so may result in patient morbidity or mortality or violate professional standards for respecting advance directives or surrogate decisionmaking. This article challenges the ethical propriety of such institutional mandates and argues that legal protections for conscientious refusal must provide redress for patients who are harmed by care that falls below the prevailing clinical standards. ‘Feminist masculinity’ might seem like a contradiction in terms. One might have assumed that we can embrace feminism or embrace masculinity, but not both. If traditional masculinity is contrary to feminist values, a pressing query for feminist men is whether repudiation of traditional masculinity should move one to reject normative masculinity entirely, or to reframe and reclaim it instead. bell hooks and Michael Kimmel each counsel against discarding manhood and masculinity. hooks envisions feminist masculinity as an alternative to patriarchal dominance, (...) with masculinity to be reconstituted in terms of love, integrity, and mutuality; Kimmel advocates moving from immature and traditional masculinities toward justice and democratic manhood, with a new model of masculinity that identifies real men with adulthood and doing the right thing. Neither proposal provides a viable stance for feminist men without collapsing into androgyny or risking erasure of some men’s and women’s identities and experiences. Building on Alcoff’s work on anti-racist whiteness, I suggest that feminist allyship practices ground a normative model of masculinity compatible with, and informed by, feminist values. We may understand allyship masculinities as open-ended feminist approaches to manhood: masculinities predicated on recognizing and responding to, rather than ignoring or accepting, the privileges and expectations distinctive of men under patriarchy. Bruno Latour is not the only scholar to reflect on his earlier contributions to science studies with some regret and resolve over climate skepticism and science denialism. Given the ascendency of merchants of doubt, should those who share Latour's concerns join the scientists they study in circling the wagons, or is there a productive role still for science studies to question and critique scientists and scientific institutions? I argue for the latter, looking to postpositivist feminist philosophy as exemplified by Alison (...) Wylie and Lynn Nelson, among others, as a guide. Feminist philosophers of science who ground their analysis in a detailed understanding of scientific practice are not science's champions nor its antagonists, but they do stand in a distinct relationship to science. If not merchants of doubt, are they scientific gadflies or perhaps in scientific loyal opposition? Though these notions can underwrite useful approaches to science studies, neither captures the distinctive interdependency and interestedness of feminist philosophers and science. I suggest that we would be better served by the notion of trustworthy science criticism, building on the analyses of trust and trustworthiness by Annette Baier, among others, attendant to the dynamics of interdependency in trust relationships. Geven, Ben‐Shakhar, Kindt and Verschuere point out that research on deception detection usually employs instructed cheating. They experimentally demonstrate that participants show slower reaction times for concealed information than for other information, regardless of whether they are explicitly instructed to cheat or whether they can freely choose to cheat or not. Finding this ‘cognitive signature of lying’ with self‐initiated cheating too is argued by the authors to strengthen the external validity of deception detection research. [75].

Douglas Diekema influentially argues that interference with parental decisions is not in fact guided by the child’s best interests, but rather by a more permissive standard, which he calls the harm principle. This article first seeks to clarify this alternative position and defend it against certain existing criticisms, before offering a new criticism and alternative. This ‘harm principle’ has been criticized for (i) lack of adequate moral grounding, and (ii) being as indeterminate as the best interest standard that it seeks (...) to replace. I argue that these are not serious problems. I take Diekema’s negative point to be right—our actual standard for intervention is not literally the best interests of the child—but I disagree with his proposed replacement. First, Diekema’s proposed harm threshold should be more carefully distinguished from Mill’s harm principle. Second, there is no reason to assume that the standard for permissible intervention coincides with the threshold for harm (or serious harm). Thus, I propose that the best alternative to the best interests standard is not a harm principle, but rather a sufficiency threshold between adequate (or ‘good enough’) and inadequate (or ‘substandard’) parenting. I propose that a feminist analysis of contemporary conversations on normalfunctioning and functional-diversity approaches to understanding disability can locate in some people’s resistances to disability an attitude compatible with respect for functional diversity. The history of feminist work in collaboration with transgender work offers an evocative model for an approach to disability that enables solidarity with those seeking functional alteration. This approach provides one way to understand how a critical analysis is compatible with respecting bodily functional desires, such that one (...) may seek physical change while working for social change alongside those successfully functioning otherwise. Is love best when it is fresh? For many, the answer is a resounding “yes.” The intense experiences that characterize new love are impossible to replicate, leading to wistful reflection and even a repeated pursuit of such ecstatic beginnings. Aaron Ben-Ze’ev takes these experiences seriously, but he’s also here to remind us of the benefits of profound love—an emotion that can only develop with time. In The Arc of Love, he provides an in-depth, philosophical account of the experiences that arise (...) in early, intense love—sexual passion, novelty, change—as well as the benefits of cultivating long-term, profound love—stability, development, calmness. Ben-Ze’ev analyzes the core of emotions many experience in early love and the challenges they encounter, and he offers pointers for weathering these challenges. Deploying the rigorous analysis of a philosopher, but writing clearly and in an often humorous style with an eye to lived experience, he takes on topics like compromise, commitment, polyamory, choosing a partner, online dating, and when to say “I love you.” Ultimately, Ben-Ze’ev assures us, while love is indeed best when fresh, if we tend to it carefully, it can become more delicious and nourishing even as time marches on. ʻEdut be-Hosef -- Derekh ʻets ha-ḥayim -- Refuʼat ha-nefesh -- ha-Tapuaḥ.

While cancer survivors commonly experience fear and anxiety, a substantial minority experience an enduring and debilitating fear that their cancer will return; a condition commonly referred to as fear of cancer recurrence. Despite recent advances in this area, little is known about FCR among people from Indigenous or other ethnic and racial minority populations. Given the high prevalence and poor outcomes of cancer among people from these populations, a robust understanding of FCR among people from these groups is critical. The (...) current review identified and aggregated existing literature on FCR amongst adult cancer survivors from Indigenous and minority populations. The protocol of this review was registered with PROSPERO in July 2020. A systematic search of bibliographic databases was conducted for relevant articles published from 1997 to November 2019. Data from eligible articles were extracted and appraised for quality by two independent reviewers. Nineteen articles from four countries met the inclusion criteria, including 14 quantitative, 4 qualitative and 1 mixed-methods study. Only one article reported on an Indigenous population. Few studies reported on FCR prevalence or severity. While the variation in tools used to measure FCR hindered a robust estimate of severity, results suggested some differences in FCR severity between minority and dominant populations, although these may have been due to study metholodological differences. Few factors were reported as being associated with FCR in minorities across multiple studies. The qualitative synthesis found five themes associated with the lived experience of FCR: variations in the lived experience of FCR; spirituality and worldview impacting on FCR; the importance of staying positive; complexities around support; and increasing cancer knowledge. The findings of this review highlight differences in FCR across cultures and contexts, which reinforces the need for culturally-specific approaches to this condition. The dearth of research in this area is of concern given the significant burden of cancer in these populations. A deeper understanding of this condition among Indigenous and minority populations is critical to developing and delivering appropriate and effective psychosocial care for cancer survivors from these groups. Miller has recently argued that fetuses have the same inherent value as non-disabled adults. However, we do not need to postulate some property possessed equally by all humans, including fetuses, in order to explain the equality of non-disabled adults. It would suffice if there were some property possessed by all non-disabled adults, but not by fetuses.

This article is about the potential justification for deploying some form of affirmative action (AA) in the context of healthcare, and in particular in relation to the pandemic. We call this Affirmative Action in healthcare Resource Allocation (AARA). Specifically, we aim to investigate whether the rationale and justifications for using prioritization policies based on race in education and employment apply in a healthcare setting, and in particular to the COVID-19 pandemic. We concentrate in this article on vaccines and ventilators because (...) these are both highly scarce resources in the pandemic, and there has been a need to develop policies for allocating them. However, as will become clear, the ethical considerations relating to them may diverge. We first set out two rationales for AAs and what they might entail in a healthcare setting. We then consider some disanalogies between AA and AARA, as well as the different implications of AARA for allocating ventilators as opposed to vaccines. Finally, we consider some of the practical ways in which AARA could be implemented, and conclude by responding to some key objections. (shrink)

Heidegger's Being and Time is an underappreciated venue for pursuing work on the role narrative plays in self‐understanding and self‐constitution, and existing work misses Heidegger's most interesting contribution. Implicit in his account of Dasein (an individual human person) is a notion of the narrative self more compelling than those now on offer. Bringing together an adaptive interpretation of Heidegger's notion of “thrown projection”, Wolfgang Iser's account of “the wandering viewpoint”, and more recent Anglo‐American work on the narrative self, I argue (...) that we read our ongoing existences in the same way that, mid‐story, we read a narrative. Reading is a better master metaphor than authorship, narration, plot, or character to guide investigations of narrative's relation to the self. It is not merely a metaphor, however, as the hermeneutic structures involved in interpreting existence and a narrative from the middle are the same. (shrink)

In an earlier article, I argued that David Estlund's notion of ‘normative consent’ could provide justification for an opt-out system of organ donation that does not involve presumptions about the deceased donor's consent. Where it would be wrong of someone to refuse their consent, then the fact that they have not actually given it is irrelevant, though an explicit denial of consent may still be binding. My argument has recently been criticised by Potts et al, who argue that such a (...) policy would involve taking organs from people whose organs should not be taken and would be a recipe for totalitarianism. The present response seeks to rebut both the ethical and political objections. I argue that people can indeed be under a moral obligation to donate their organs, even if they are not technically dead at the time and their donation does not save anyone else's life. Moreover, I argue that an opt-out system—unlike mandatory donation—is not totalitarian because it preserves the right of individuals to act morally wrongly, by opting out when they have no good moral reason to do so. Eric Davidson was a legend both in his science and his personality. He inspired and challenged a new generation of developmental biologists and I was lucky to be one of them. He changed the way we think about biological interactions by synthesizing a large scale, almost incomprehensible set of data into a causal model of a gene regulatory network. While his death leaves a big hole in our lives, his contribution to the conceptualization of regulatory biology will inspire developmental and (...) evolutionary biologists for decades to come. RésuméDans cet article, j'analyse des passages cruciaux de la composition finale du théologien Faḫr al-Dīn al-Rāzī, Al-Maṭālib al-῾āliya, afin de théoriser sur la nature de deux de ses études : la cosmologie et l'angélologie. En cherchant à prouver l'existence de ces êtres, Rāzī divise la réalité en deux domaines : matériel et intelligible. Les anges, qui symbolisent les intellects et les sphères, existent dans une réalité intelligible comme des êtres qui n'occupent pas d'espace. Parmi eux, certains sont associés aux corps (...) célestes, et d'autres ne s'associent à aucune chose matérielle. Dans un deuxième temps, je présente des arguments suggérant que Rāzī pensait peut-être que ces êtres célestes sont prééternels avec Dieu. Ces conceptions montrent une continuité avec la tradition philosophique islamique, et elles la structure philosophique de la réalité intégrée par al-Ġazālī dans quelques textes théologiques. De plus, on a examiné d'autres courants philosophiques développés dans le monde islamique qui ont pu agir sur la cosmologie de Rāzī, en lien avec la tradition hermétique et avec les Épîtres des frères de la Pureté.
